ABSTRACT

With deepening understanding of surgical treatment of calcaneal fractures,clinical applications of external fixators have drawn more attention.At present,however,there has been no systematic review of such applications for intra-articular calcaneal fracture.This review collected all the recent reports available on such applications to analyze the use and clinical indications of different external fixators.Moreover,on the basis of our own clinical experience,we further proposed a novel treatment protocol for intraarticular calcaneal fractures which integrates internal and external fixation and minimally invasive techniques.

ABSTRACT

As a pivotal part of the elbow joint structure,coronoid process of the ulna plays a vital role in maintaining stability of the elbow joint.Loss of coronoid process height probably causes instability of the elbow joint.Diagnosis and treatment of coronoid process fracture is a widespread concern for orthopaedic surgeons.Nevertheless,reconstruction of the defects following coronoid process fracture has been rarely reported though they affect elbow stability.It is very challenging to treat the elbow instability induced by coronoid process defects,mostly complicated with other elbow injuries.Moreover,the clinical treatment efficacy remains elusive.This review summarizes the research advances in reconstruction of coronoid process defects.

ABSTRACT

The practice of daily case discussion in the morning using slice show has been cartied out since orthopedic trauma department of Nanfang Hospital was set up.Bilingual ( Chinese and Englisl) discussion was hold in the department before and after the operation on the cases requiring three-level or higher level operation and those of special diseases.The bilingual case discussions will greatly improve the clinical thinking and accumulate the clinical experiences for doctors.The professional English proficiency of all doctors in the department have been improved because of the bilingual case discussions and the most effective treatment for patients was provided at the same time.

ABSTRACT

Objective To investigate the injury mechanism, clinical classification, treatment methods and clinical results of calcaneal fractures. Methods From June 2005 to May 2009, 83 feet in 70 patients with calcaneal fractures were treated. According to Sanders and Essex-Lopresti classifications, 63 feet had intra-articular fractures (30 feet of type Ⅱ, 21 type Ⅲ, and 12 type Ⅳ ) and 20 feet had extra-articular fractures (3 feet of lateral condylar fracture, 5 anterior process fracture, 9 calcaneal body fracture and 3 nodules fracture) . Reduction with a conventional lateral L-incision, mini-incision, or leverage was conducted depending on the clinical classification of the fracture involved. Fixation with a conventional plate or locking plate, simple screws and Kirschner wire was applied in all patients. Bone grafting was performed in the 20 cases whose defects were larger than 2 cm3. Functions and complications of the feet were observed at the follow-up. Results The 70 patients were followed up from 12 to 30 months (average, 18 months). The total average American Orthopaedic Foot and Ankle Society(AOFAS) score was (90.3 ± 11.0) points (48 to 100 points). The average AOFAS score for the 20 feet of extra-articular calcaneal fracture was (98.1±2.6)points (90 to 100 points), with no complications; the average AOFAS score for the other 63 feet of intra-articular calcaneal fracture was (87.8 ± 11.4) points (48 to 100 points) with such complications as necrosis at the skin edge of incision in 2 feet, sural nerve injury in 4 and traumatic subtalar arthritis in 10.Conclusions Treatment of extra-articular calcaneal fractures may have better clinical results than treatment of intra-articular calcaneal fractures. The preoperative evaluation of the fracture type and severity of soft tissue injury is especially essential to the treatment.

ABSTRACT

Objective To investigate the operative techniques and clinical significance in the treatment of large area soft tissue defect in children limbs by sequential vacuum-assisted closure and free flaps. Methods Twenty-two cases of children patients, with a range between 3 and 10 years, were treated by sequential vacum-assisted closure (VSD) and free flaps. All the patients suffered from large area soft tissue defect after severe trauma of limbs, with bone, tendon and other deep tissues exposed or partial defected. The wound area varied from 12 cm × 8 cm to 34 cm × 25 cm, and among the wound 9 cases were fresh and the other 13 were necrotizing infection. After strictly cleaned, the wound was covered with VSD according to the condition of the soft tissue defect and its underlying tissue damage. For the patients complicated with unsteady fracture, the fracture was fixed with outside-fixed frame or inside-Kirschner nails. The VSD negative pressure membrane was removed after 5 to 9 days, then the wound was repaired by free flaps grafting in 8 cases and by free flap combined with skin grafting in the other 14 cases. The survival and recovery conditions were followed up after the operation. Results After treatment by VSD, no obvious infection was seen in 22 cases of children patient with large area soft tissue defect complicated by underlying tissue exposure, and granulation tissue surrounding tendons and bone exposure area grew well. The survival rate of the free flaps was 100% whithout vascular crisis.The infection was well controlled, and no fistula tract was seen. The patients were followed up for 6-24 months and the flaps had soft texture and good appearance with satisfactory functional recovery of the affected limbs.Conclusion For pediatric patients with large area soft tissue defect complicated by underlying tissue exposure, after strict debridement, the treatment circle will be obviously shortened by the application of sequential vacuan seding dranage and microsurgical technique, and the success rate of the operation is improved. Therefore, function of the affected limbs in children will be recovered as far as possible.

ABSTRACT

Objective To report our first experience of applying the percutaneous compression plating (PCCP) for treatment of femoral intertrochanterie fractures. Methods On July 22, 2008, a 79-year-old female was treated by percutaneous compression plating for her pertrochanteric hip fracture. The patient was first placed supine on the traction table for closed fracture reduction. Under the guidance of C-arm radiography, stab incisions were made and the plate was put appropriately. Next 2 neck screws and 3 shaft screws were inserted in proper order. The patient was followed up every month after operation. Comparisons were made between the literature experiences available and ours. Results The reduction of in-tertrochanteric fracture was anatomic. The retention of PCCP was fine. The time for PCCP was 90 rain, and intraoperative hemorrhage was about 100 ml. Follow-up for 3 months showed that functions of the hip were excellent according to Harris scoring (91 points;extension to flexion: 0~150°). Conclusion PCCP is a safe and effective way for treating intertrochanteric fractures, for it leads to reliable fixation, less invasion, and .good recovery of hip function.

ABSTRACT

BACKGROUND: It has been reported that in China, human bone marrow mesenchymal stem ceils are mostly harvested from adults. Studies on bone marrow mesenchymal stem cells in children are few.OBJECTIVE: To isolate and expand bone marrow mesenchymal stem cells from children, and to analyze the biological characteristics of bone marrow mesenchymal stem cells and their potential of differentiating into osteoblasts, adipocytes and neural like cells.DESIGN: Observational comparative study.SETTING: Tongji Medical College, Huazhong University of Science and Technology.MATERIALS: Experiments were performed at the Laboratory of Department of Orthopaedics of Wuhan Tongji Hospital from March to September 2006. Bone marrow mesenchymal stem cells were collected from one boy patient and two girl patients aged 5-8 years, who received pelvis osteotomy for dysplasia of the hip joint. The experimental procedures were approved by the Hospital Ethics Committee and family members of all children patients singed the informed consent.Dexamethasone, vitamin C, β-sodium glycerophosphate, 3-1sobutyl-1-methylxanthine, insulin, indometacin and butylated hydroxyanisole were bought from Sigma Company. Dimethyl sulphoxide was purchased from Amersco Company.METHODS: Bone marrow mesenchymal stem cells were cultured from mononuclear cells isolated over a Percoll gradient.Bone marrow mesenchymal stem cells were observed under an inverted phase contrast microscope. Bone marrow mesenchymal stem cells could differentiate into osteoblasts, adipocytes and neural like cells with osteoblast inductor (β-sodium glycerophosphate, dexamethasone, vitamin C), lipoblast inductor (dexamethasone, 3-isobutyl-1-methylxanthine,bovine insulin, indometacin) and serum-free medium inductor (dimethyl sulphoxide, butylated hydroxyanisole) respectively.Osteoblast marker (alkaline phosphatase, osteocalcin mRNA, calcium node), adipocyte marker (lipid droplet, PPAR γ-2mRNA) and neural ceil-like marker (nissl body, neuron specific enolase, neurofilament protein) were respectively determined by the immunohistochemical method, polymerase chain reaction and immunocytochemical method.MAIN OUTCOME MEASURES: ①Appearance and proliferation of bone marrow mesenchymal stem ceils from children,and ②determination results of osteoblast, adipocyte and neural cell markers.RESULTS: ①Children bone marrow mesenchymal stem cells could easily adhere to the wall, appeared fusiform, had high reproductive activity and arranged vortically after fusing. ②Appearance of bone marrow mesenchymal stem cells changed after receiving inductor. Osteoblast marker, adipocyte marker and neural cell-like marker were positive after chemical staining, polumerase chain reaction and immunocyte staining.CONCLUSION: Children bone marrow mesenchymai stem cells show stable proliferation, passage and multi-direction differentiation towards osteoblasts, adipocytes and neural like cells.

[Objective]To study the effects of verapamil on the immediate-early gene(IEGs)expression of bone marrow mesenchymal stimulated by mechanical strain,and deduce the effects of calcium ion channel on the early responses of children bone marrow mesenchymal stem cells(MSCs)to mechanical strain in vitro.[Method]MSCs were isolated and cultured.The 3-6th generational MSCs were stimulated in different time by mechanical and(or)verapamil(20?mol/l).Flow cytometry was applied to measure intracellular free Ca2+ at once.Early-response genes /proteins(c-fos,c-jun and c-myc)were examined by RT-PCR and immunocytochemical stain.[Result]The application of mechanical strain increased intracellular c-fos,c-jun and c-myc mRNA /protein levels.The resulting effect is partially inhibited before MSCs were preincubated with verapamil(P
